Our responsibilities
Downey Outpatient Surgery Center is required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, and notify you of any breach of unsecured PHI.
Uses and disclosures of PHI
- Treatment — to provide, coordinate, or manage your surgical care and related services.
- Payment — to obtain payment for treatment, including billing insurers, workers’ comp carriers, and lien-based arrangements.
- Health care operations — for quality improvement, credentialing, accreditation, audits, and training.
- Required by law — for public health reporting, legal process, and other uses required or permitted by law.
Your rights
You have the right to inspect and obtain a copy of your PHI, request corrections, request restrictions on certain uses, request confidential communications, receive an accounting of disclosures, and obtain a paper copy of this Notice.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at [email protected] or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
